I have recently taken on the task of adapting the Dark Souls video game into a tabletop card game.  Dark Souls is known for its extreme difficulty, in depth mythology, and unique use of gaming conventions.  The card game focuses on promoting cautious progression by the player through a number of changing rules and mechanics that make each challenge a unique experience.  The difficulty in creating the card game comes from moving game conventions like battle momentum, maintaining the mythology, and allowing success with various play styles.
